I, the undersigned, hereby irrevocably consent to and authorize Vancouver Youth Advocacy Society (“VYAS”) to use, reproduce, exhibit, display, broadcast and/or distribute images of mine (including any video or photographs of my likeness, poses, acts and appearances or sound records of my voice taken or made by VYAS or its contractor(s) or media partners and sponsors (“Images”) via any media now known or later developed for any purposes in connection with promotion, marketing or record of VYAS and/or its activities (which may include printed or electronic publications or advertisements, research or presentation materials, websites, social media, newspaper, TV or radio broadcast). I agree that the copyright and other intellectual rights in the Image shall belong to VYAS. I also understand that personal data, including the Images, may be collected in the course of my participation in VYAS activities. I agree that VYAS may crop, alter and modify the Images and combine such Images with other images, text, audio recordings and/or graphics, and/or amend or edit any materials containing the Images or my personal information without further notice.

本人给予不可撤回的同意及授权予温哥华青少年互助协会（简称“青助会”）使用、复制、展示、显示、广播及/或分发本人之影像(包括青助会或其合同方或媒体合作方或赞助商，就本人形象、姿态、动作、外观的摄取或制造的照片或影片或其他纪录或就本人声音之记录)(下 称“该影像”)。该影像可用于任何现有或未来的合作媒体作有关于宣传、推广及/或报导青助会/或者其活动的用途（可能包括任何印刷或电子出版物或广告、研究报告或演说的材料、互联网、社交网络媒体、报刊、电视或电台广播）。本人同意该影像的版权及其他知识产权属青助会所有。 本人亦明白于本人参与该活动期间青助会或其合同方或媒体合作方或赞助商可能收集本人的个人资料(包括该影像) 。本人同意青助会如需剪裁、改变、修饰该影像及／或把该影像与其他影像文字、声音及/或图像组合，及/或需要不时修改和编辑任何包含该影像的材料，不用另行通知。
